Our Community Partners
The true success of the Caring for Our Community (CFOC) initiative lies in the powerful partnerships that have been forged along the way. These collaborations form the backbone of the initiative, bringing together a diverse network of organizations, healthcare providers, and community leaders, all united by a shared mission: to uplift and empower the most vulnerable among us.
Our partners don’t just support CFOC; they are integral to its success. They operate in multiple capacities—whether by providing direct services, offering resources, or helping to identify gaps in care that need to be filled. Each partner plays a critical role in ensuring that the individuals we serve have access to comprehensive support, from immediate assistance to long-term solutions.
Together, we’ve built a web of care that extends across the entire community, connecting people to food, housing, transportation, and much more. These partnerships amplify our impact, allowing CFOC to not only reach more people but also to provide deeper, more sustained help. By coming together with a shared vision, we are breaking down silos and proving that true change happens when we work hand in hand, supporting each other in the journey toward a healthier, more equitable future for all.

All Care Health Center
Working with you to stay healthy. Our providers believe in a patient-centered, team-based approach to care. The team consists of you, your health care provider, other health care professionals and trusted friends and family members. This team works together to help make the best choices about your health
Anawim Housing
At Anawim, we work to ensure the most vulnerable always have a safe place to call home, and we are committed to ending homelessness. Anawim Housing provides over 390 individuals who have experienced homelessness with permanent supportive housing. Individuals in our programs come to us from long-term homelessness and live with a qualifying disability: mental illness, substance use disorder, physical disability, diagnosis of HIV/AIDS, or are fleeing domestic violence.

Connections Area Agency on Aging
Connections Area Agency on Aging is proud to serve individuals and families in western, southwestern, and south-central Iowa as they seek solutions and strive for independence and dignity.
Council Bluffs Fire Department
The Council Bluffs Fire Department (CBFD) provides a full range of life safety protection for the 60,000 plus residents who live in Iowa's eighth-largest city. The CBFD's 108 career personnel, including firefighters and paramedic-trained personnel, give emphasis to all aspects of life safety. This includes fire prevention, fire fighting, emergency medical care, technical rescue, hazardous materials mitigation, disaster response, public education, and community service.
Council Bluffs Police Department
A Message from the Chief
"The dedicated men and women of the Council Bluffs Police Department serve over 62,000 residents providing 24/7 community-based police services. Our 118 sworn and 22 civilian employees work diligently to leverage available resources and partnerships to enhance our ability to provide top quality service, build and strengthen relationships with all our citizens, and to most effectively address crime and social issues."
Glenwood Fire Department
Established in 1876, the Glenwood Fire Department has a long, rich history of providing Fire and EMS services to the citizens of Glenwood and Mills County. The Glenwood Fire Association has grown and evolved into paid 24/7 ALS EMS service and a highly progressive and aggressive volunteer fire service with a Class 4 ISO rating that responds to approximately 2,000 calls annually.

Heartland Family Service
Strong Communities, So Everyone Can. Founded in Omaha in 1875, Heartland Family Service connected with nearly 120,000 individuals and families through direct services, advocacy, education, and outreach last year, from 20 locations in east central Nebraska and southwest Iowa. Our services provide critical human services to the individuals and families who ultimately shape the future
of our community in the following program focus areas: Housing, Safety, & Community Well-Being, and Counseling & Prevention.
Methodist Jennie Edmundson Hospital
Methodist Jennie Edmundson Hospital has served the health and medical needs of southwestern Iowa since 1886. We have the only accredited cancer program in southwest Iowa. Our hospital is recognized for excellence in key quality measures for heart attack, heart failure, pneumonia, and surgical care.
Mills County Public Health
Our mission is to enhance the quality of life within our county by promoting healthy behavior, protecting the environment, and preventing disease. The members of our health-care team provide a variety of services, which are based on a public/private partnership.
New Visions Homeless Services:
New Visions exists with the mission to “provide help, hope, and opportunity for those experiencing hunger and homelessness in the Council Bluffs and Omaha metro.”
Pottawattamie County Public Health
Pottawattamie County Division of Public Health’s mission is to work in partnership to protect all of Pottawattamie County residents’ health. Health; that optimal state of physical, mental, and environmental well-being that is a right and responsibility of all residents of Pottawattamie County.
Southwest Iowa Mental Health and Disability Services Region
The region's plan is designed to improve health, hope, and successful outcomes for the adults in the region who have mental health disabilities and intellectual/developmental disabilities, including those with multi-occurring substance use issues, health issues, physical disabilities, brain injuries, and other complex human service needs. It is our mission "to provide a responsive and welcoming system of support for overall mental wellness".
Threshold
Centering safe and accessible housing as a human right through unified action. Threshold CoC coordinates the collective response to housing justice for Douglas, Sarpy, and Pottawattamie Counties. We lead a coalition of community partners who work directly with people experiencing homelessness, allies who amplify our work, and change agents who advocate for policy reform. We work together to ensure homelessness is rare, brief, and non-recurring.
Together
Together's mission is to prevent and end hunger and homelessness. This is done through strategic partnerships and collaborations with other nonprofit organizations, government, businesses and caring individuals. Together's core values of hope, dignity, compassion, excellence, and diversity is the guiding force in how the organization leads and serves those who need Together most. Together envisions a community of prosperity where everyone experiences safe, affordable housing, food security, health and wellness.
