Meet Our CFOC Community Health Coaches
Across southwest and western Iowa, our Community Health Coaches help people connect with healthcare services and local supports that strengthen health and stability.
Through outreach in neighborhood and healthcare settings, partnerships with local organizations, and one-on-one support, our Health Coaches help individuals navigate healthcare services, connect with essential resources, and take meaningful steps toward better health and well-being.
Each Community Health Coach brings unique experience, strong local relationships, and a deep commitment to serving others. Below, you can learn more about each coach, including where they serve and how they support individuals and families across our region.
Ben Berberich
First Responder Community Health Coach - Mills County
Ben serves as a Community Health Coach supporting Mills County residents through the Caring for Our Communities program. He connects community members to essential resources that address the social drivers of health — including access to food, transportation, medications, housing stability, follow-up care, and community supports. By assessing individual needs, coordinating referrals, and providing hands-on assistance, Ben helps reduce barriers to care and supports individuals in improving their overall health and well-being.
Ben works closely with local partners and community organizations to strengthen connections and improve access to services across Mills County. In rural communities where distance, transportation, and limited resources can make navigating systems difficult, he helps coordinate care and ensure individuals are connected to the right support at the right time.
Ben is passionate about walking alongside community members and helping ensure no one has to navigate challenges alone, supporting healthier and more connected communities across Mills County.
Jeanna Bull
Community Health Coach - Connections Area Agency on Aging
Jeanna serves as a Community Health Worker embedded with Connections Area Agency on Aging, with her role supported through the Caring for Our Communities program. She works with adults aged 60 and older, as well as their families, helping them navigate healthcare needs following an emergency room visit or hospital stay. Jeanna helps identify barriers related to the social drivers of health — such as access to food, transportation, medications, housing stability, follow-up care, and community supports — and connects individuals to resources that support their health and independence.
Jeanna works closely with healthcare teams, community organizations, and aging services partners to ensure older adults and their families have access to the support they need during critical transitions in care. Through education, coordination, and ongoing support, she helps individuals feel confident navigating healthcare systems and community resources.
Jeanna is passionate about helping people feel supported, know who to call when challenges arise, and never feel alone in their health journey. As the first Community Health Worker in the Caring for Our Communities program, she has spent more than a decade helping build systems of support that improve access to care and strengthen community connections.
Sydney Clair
Community Health Coach - Behavioral Health, Methodist Jennie Edmundson Hospital
Sydney serves as a Community Health Worker embedded within the Behavioral Health team at Methodist Jennie Edmundson Hospital, supporting individuals who frequently utilize inpatient behavioral health services. Through the Caring for Our Communities program, Sydney conducts assessments on the behavioral health unit, supports case management efforts, and helps plan safe, thoughtful discharges that set patients up for success beyond their hospital stay.
Sydney works closely with hospital staff, community organizations, and fellow Community Health Workers to coordinate referrals, connect patients to community-based resources, and provide follow-up support after discharge. She also leads educational groups focused on coping skills, helping patients build practical tools they can use in their daily lives.
Sydney is passionate about helping individuals feel supported, build confidence in managing their health, and stay connected to the care and resources they need after leaving the hospital.
Chasity Kephart
First Responder Community Health Coach - Council Bluffs Police Department
Chasity Kephart serves as the First Responder Community Health Coach, working at the intersection of public safety, healthcare, and community support. Through the Caring for Our Communities program, Chasity is embedded within the Council Bluffs Police Department, where she receives referrals from the Fire Department, CBPD, and community partners for individuals experiencing social drivers of health needs. She helps assess needs, coordinate referrals, and connect individuals to resources that support stability, safety, and overall well-being.
Chasity works closely alongside the Homeless Outreach Program (HOP) and participates in regular, in-community outreach to connect with individuals where they are. Through relationship-based engagement, she provides resources, referrals, and ongoing support to individuals who may otherwise face barriers to accessing services.
Chasity is passionate about walking alongside individuals from the moment of crisis through connection and coordinated support, remaining engaged until greater stability is achieved.
Madison Mahoney
Community Health Coach - Methodist Jennie Edmundson Hospital
Madison serves as a Community Health Worker based at Methodist Jennie Edmundson Hospital through the Caring for Our Communities program. She supports patients under age 60 who have housing and are transitioning from the hospital back to their home. Madison helps connect and coordinate resources, reduce barriers to care, and serves as a trusted point of contact for patients during this critical transition period.
Madison works closely with patients, healthcare teams, and community partners to ensure individuals have access to essential supports such as food, transportation, medications, follow-up care, and other community resources that support long-term stability and health.
Madison is passionate about bridging the gap between medical care and the support systems people need to thrive, helping ensure patients feel supported not only during their hospital stay but long after they return home.
